Select records to retrieve based on the type of test performed and/or the state in which the test site is located. Select one or more of the test types along with none, one, or more states, and then click on the QUERY button. To select more than 1 item from the list, hold down the Control (Ctrl) key and click on the items. If you do not want to limit the records by state, do not select any states or select all of the states.
